What is WDEIA or exercise-induced wheat allergy, its symptoms, diagnosis and treatment and how is it distinguished from coeliac disease?


Exercise-Dependent Wheat Allergy (WDEIA): Diagnosis and Management

Wheat allergy is an immune reaction to proteins found in wheat. In some patients, symptoms do not appear following isolated consumption, but only when this is combined with physical exercise.

This condition is known as wheat-dependent exercise-induced anaphylaxis (WDEIA). It is a rare but potentially serious condition, which requires a high degree of clinical suspicion for diagnosis.

Mechanisms involved in wheat-dependent exercise-induced allergy

WDEIA is a form of IgE-mediated allergy in which the ingestion of wheat alone does not usually trigger symptoms. It is the combination with a co-factor - usually exercise - that precipitates the reaction.

From a pathophysiological perspective, several mechanisms have been proposed:

  • Activation of tissue transglutaminase (tTG) during physical stress, promoting the formation of antigenic complexes with a greater capacity to bind to IgE.
  • Increased intestinal permeability during exercise, which facilitates the passage of allergens into the circulation.
  • A decrease in the activation threshold of mast cells and basophils.

The main allergens involved are omega-5 gliadin and high-molecular-weight glutenins, which play a key role in the allergic response.

Risk factors for exercise-induced wheat allergy

There are several factors that may predispose individuals to the development of this condition:

  • Personal or family history of atopy (asthma, rhinitis, food allergies).
  • Paediatric age, at which wheat allergy is more common, although it may persist or develop in adults.
  • Possible cross-reactivity with grass pollens in some cases.

In childhood, wheat allergy usually resolves over time, but in adults it may present in a more persistent form or in specific contexts such as WDEIA.

Clinical symptoms of WDEIA or exercise-induced wheat allergy

Clinical symptoms of WDEIA or exercise-induced wheat allergy The key characteristic of this condition is that symptoms appear when wheat consumption coincides with physical exercise, generally between 1 and 4 hours later.

WDEIA: symptoms and clinical manifestations

The clinical spectrum is broad and includes:

  • Skin symptoms: hives, angioedema.
  • Gastrointestinal symptoms: abdominal pain, nausea, vomiting, diarrhoea.
  • Respiratory symptoms: nasal congestion, dyspnoea.
  • Oropharyngeal symptoms: itching or swelling in the mouth and throat.
  • Cardiovascular involvement: hypotension.

In its most severe form, it can cause anaphylaxis, a potentially life-threatening systemic reaction requiring urgent treatment.

Diagnosis of exercise-induced wheat allergy

The diagnosis of WDEIA is clinical and immunological, and can be complex due to the variability of the factors involved. It is based on several key elements:

  • A detailed medical history, identifying the relationship between wheat intake, exercise and the onset of symptoms.
  • Skin prick tests with wheat extracts.
  • Measurement of specific IgE, particularly against omega-5 gliadin, which has high sensitivity and specificity.
  • Controlled provocation test with wheat and exercise, considered the gold standard, although it is carried out in specialised settings.

There is often a delay in diagnosis, as it can be confused with other conditions such as exercise-induced anaphylaxis or acute urticaria, so a high degree of clinical suspicion is required.

Treatment and prevention of wheat and exercise allergy reactions

Management of this type of wheat allergy is based primarily on preventing combined exposure to the triggers.

Among the strategies that are usually most effective are:

  • Avoiding the consumption of wheat before physical exercise.
  • Maintaining a minimum interval of around 4 hours between consumption and exercise.
  • In severe or high-risk cases, consider a completely wheat-free diet.

In addition, it is important to properly inform the patient about the risk of anaphylaxis and, where indicated, ensure that they have access to emergency treatment.

Differential diagnosis compared to coeliac disease and other reactions to wheat

Finally, it is important to remember that it is essential to distinguish wheat allergy from other conditions related to this cereal:

  • In wheat allergy, the reaction is immediate or IgE-mediated.
  • In coeliac disease, it is a chronic autoimmune condition triggered by gluten in predisposed individuals.
  • In non-coeliac gluten sensitivity, there is no involvement of a classic allergic mechanism or a defined autoimmune response.

Unlike in coeliac disease, patients with wheat allergy or WDEIA usually tolerate other gluten-free cereals.
